HEBER CITY, Utah-A head-on collision on U.S. 189 Sunday afternoon claimed a life per a report from the Utah Highway Patrol.

UHP confirms the incident occurred at 3000 South in Heber City just before 3:30 pm.

Cpl. Michael Gordon of UHP reported a pickup truck was traveling southbound when, for an unknown reason, he crossed into northbound traffic. He struck another vehicle head-on, resulting in his death.

In news revealed Monday, five who were injured in the incident, were taken to the hospital.

Those who survived the incident were not deemed to have life-threatening injuries per subsequent reports.
