HEBER CITY, Utah-Per an official statement from Wasatch High athletic director Steve Marsing, the day for free physicals for Wasps athletes will be June 19 at Wasatch High School.
This will be on the west campus at the south gym from 8:00 am-12:00 pm. To sign up, please follow this link: https://www.signupgenius.com/go/5080948a8ae2fa6f85-wasatch1 to get a time slot for your physical or call 657-4690.
Athletes need to bring a copy of the physical form which can be found at www.uhsaa.org/forms/forma.pdf.
Please fill out the disclosure/consent portion as well as the history form before showing up for your time slot.
If athletes fail to sign up for a time slot beforehand, Wasatch High cannot guarantee them a physical at this time.
Parents/guardians of student-athletes should confirm with Wasatch High officials any past health issues their children may have had. Additionally, if they have questions/concerns for the doctor, they are to set up an appointment with primary care physicians.
