A gray 2003 Honda Odyssey van occupied by a family of 4 was traveling northbound near the area of milepost 82 per a report released Sunday afternoon. The right rear tire of the van experienced a failure, causing the driver to lose control. The van then traveled into a median, rolling over twice. 14-year-old Zahara Ismael and 74-year-old Hamdiyan Salman, each females, were riding in the second row seat and were not restrained.
As the van rolled, each of them were ejected. Ismael sustained fatal injuries at the scene and Salman sustained severe injuries and was later transported to a hospital to meet a medical helicopter for further transport.
Salman died from her injuries at the hospital. The female driver and front-side male passenger were restrained and received non-life-threatening injuries. They were transported to area hospitals for treatment. At this time, traffic was rerouted off of northbound Interstate 15 at the Paragonah exit to bypass the crash scene. This detour lasted for approximately 2.5 hours. impacting traffic minimally.
An inspection of the vehicle was completed on-scene, revealing the vehicle had multiple equipment violations and was not safe to drive on the roadway.

UHP confirms Interstate 15 is back open at milepost 82.
I-15 is now open at mile post 82.

Sunday morning, a crash occurred on Interstate 15 at milepost 82, near the exit to Paragonah in Iron County.
The Utah Highway Patrol confirmed the crash occurred at 7:25 am.
I-15 is shutdown in both directions due to a fatal crash near mile post 82. Please use alternate routes. We are still gathering information and will post it shortly.

As of 9:57 am, UHP confirmed that northbound traffic was being diverted off the on-ramp and right back on to I-15 en route to Beaver.
Southbound I-15 is now open. Northbound is being diverted off of the on ramp and right back onto I-15.
